S-Five-T Misophonia Trigger Checklist

The S-Five-T is the trigger checklist that accompanies the S-Five misophonia assessment. It records which sounds have affected you during the past few weeks, the main feeling each sound caused, and how intense your reaction was.

This checklist does not produce a diagnostic cut-off. It creates a practical profile of the breadth, intensity, and emotional pattern of your current triggers. It may help you identify accommodations, explain your experience, or prepare for a conversation with a clinician.

How to Answer

Think about the past few weeks. For every sound, choose the main feeling it caused and rate the intensity from 0 (Doesn't bother me at all) to 10 (Unbearable/causes suffering).

If a sound has not bothered you, select No feeling and intensity 0.

    Trigger Count describes breadth, Combined Intensity reflects both breadth and intensity, and Average Trigger Intensity estimates how strong the reaction is across sounds that affect you. The authors do not publish a clinical threshold for these indices.

    The list is customizable and cannot include every possible trigger. A low Trigger Count does not mean that one severe trigger is unimportant.
